1. Size of tank? 125l / c. 33 U.S. gallons

2. Water parameters
a. Ammonia? 0
b. Nitrite? 0
c. Nitrate? 40ppm (comes out of tap like this unfortunately)
d. pH, KH and GH? pH 7.8, KH 10, GH 14 degrees (German scale)
e. Test kit? API liquid master

3. Temperature? 25.1

4. FW (fresh water) or BW (brackish)? FW

5. How long the aquarium has been set up? One month

6. What fish do you have? How many are in your tank? How big are they? How long have you had them? Six Rosy Tetras and one Bristlenose Plec, introduced last Saturday following 1-hour acclimatisation period)

7. Were the fish placed under quarantine period (minus the first batch from the point wherein the tank is ready to accommodate the inhabitants)? N/A - first batch following fishless cycle

8. a. Any live plants? Fake plants? - echinodorus amazonicus and alternanthera rosaefolia
b. Sand, gravel, barebottom? Dark sand
c. Rocks, woods, fancy decors? Any hollow decors? One piece of mopani wood, two coconut caves and some catappa leaves

9. a. Filtration? External - Fluval 207 canister (includes carbon)
b. Heater? 100W Juwel (came with the tank)

10. a. Lighting schedule? What lights are used? Lights first on 48 hours after introduction, on for c. 10 hours and off the rest of the evening/all night
b. Any sunlight exposure? How long? No direct sunlight, some natural light from nearby window but tank is in very shaded area.

11. a. Water change schedule? Planning to do first change (since fish introduced) tomorrow
b. Volume of water changed?
c. Well water, tap water, RO water? Tap water
d. Water conditioner used? Seachem Prime
e. Frequency of gravel/sand (if any) vacuumed?

12. Foods?
How often are they fed?

13. a. Any abnormal signs/symptoms? All fish currently seem active, and are feeding eagerly
b. Appearance of poop? Mainly creamy/white, some red (possibly due to food)
c. Appearance of gills? Normal

14. a. Have you treated your fish ahead of diagnosis? Ashamed to say yes
b. What meds were used? API white spot cure - one dose
