1. Size of tank? .....75gal

2. Water parameters
a. Ammonia?....0ppm
b. Nitrite?.....0ppm
c. Nitrate?.....~10ppm
d. pH, KH and GH?....7.2-7.4 pH........GH 80
e. Test kit? API, Tetra brand, and generic

3. Temperature?....... 77F cichlids, 74F goldfish

4. FW (fresh water) or BW (brackish)?....FW

5. How long the aquarium has been set up?....3 months

6. What fish do you have? How many are in your tank? How big are they? How long have you had them?
.... started with south american cichlids and congo tetras, all young (2in). Switched to African cichlids after all but tetras died. Switched to comet goldfish after cichlids all died to avoid $ loss while I figure things out (they will go in my outdoor pond in the spring). Pictus catfish went in with the africans (still alive with tetras and a few goldfish atm).

7. Were the fish placed under quarantine period (minus the first batch from the point wherein the tank is ready to accommodate the inhabitants)?

8. a. Any live plants? Fake plants?
b. Sand, gravel, barebottom?
c. Rocks, woods, fancy decors? Any hollow decors?
.....Pothos plants growing out of tank. Brand new fake plants, black caribsea sand new, rocks and wood were removed to eleminate factors. They were possibly toxic as fish deaths increased when I put them back in. However half of the fish that died where never exposed to rocks or driftwood (could there be toxins stuck in filter?)

9. a. Filtration?.... Hydor canister (400?) and aquaclear 500 with original media
b. Heater? ....yes two ehiems

10. a. Lighting schedule? What lights are used?
b. Any sunlight exposure? How long?
....white/blue leds, indirect sunlight. light on from 1-9pm

11. a. Water change schedule?
b. Volume of water changed?
c. Well water, tap water, RO water?
d. Water conditioner used?
e. Frequency of gravel/sand (if any) vacuumed?
.....Weekly or more when toxins were suspected. (~40%). Regular sand vac (scim the top for waste once/twice week). Tap with conditioner.

12. Foods?
How often are they fed?
....varied, all quality ingredients

13. a. Any abnormal signs/symptoms?
b. Appearance of poop?
c. Appearance of gills? either act normal begging for food schooling etc and drop dead or more often they go pale site at bottom of tank or in top corner before dying. Water parameters do not change! I test all the time thinking it was an ammonia spike. Sometimes gills are a tad red but rarely. Goldfish aren't the healthiest and fecals have been white and stringy. My pictus is happy and health and the congos have been flashing and mating. There is no aggression at all in the tank. Tank is in office/bedroom so I'm almost always watching it.
14. a. Have you treated your fish ahead of diagnosis?
b. What meds were used?
...aquarium salt (1tbs per instructions) roobios tea (which seems to help, just started)

Extra- fish all act healthy for a few days, then I have 1-2fish death a day till I'm back to just tetras and catfish.

Just found out that Tahitian Moon sand is toxic! Tons have had this issue. Caribsea does not sell this product any more
