1. Size of tank - 20 tall

2. Water parameters
a. Ammonia - 0ppm
b. Nitrite - 0ppm
c. Nitrate - 40ppm (water change is tomorrow, once a week, 25%
d. pH, KH and GH - not sure.
e. Test kit - API liquid

3. Temperature - 78*

4. FW

5. How long the aquarium has been set up - 5 months

6. What fish do you have? How many are in your tank? How big are they? How long have you had them?

6 Black Phantoms ranging from 1.5-2" - 4 Cory Cats about 1.5" ea. All fish have been in for about 4 months.

7. Were the fish placed under quarantine period (minus the first batch from the point wherein the tank is ready to accommodate the inhabitants)?

phantoms - yes. Corys - no.

8. a. Any live plants? Anubias, Wisteria, Amazon Sword, Ozelot Sword, Crypts, Vals
b. Sand
c. 2 pieces of drift wood

9. a. Filtration - yes, aqueon quietflow 10 (rated for up to 30 gallons)
b. Heater - aqueon 100 watt w/ temp control

10. a. Lighting schedule? What lights are used? 8am-5pm 2 t5ho
b. Any sunlight exposure? How long? no

11. a. Water change schedule - everythursday
b. Volume of water changed - 25%
c. Well water, tap water, RO water - tap
d. Water conditioner used - forgot its name at the moment can update when i am home
e. Frequency of gravel/sand (if any) vacuumed - no, was told its not neccesary for sand.

12. Foods - New life spectrum and blood worms
How often are they fed - every other day, bloodworms once a week

13. a. Any abnormal signs/symptoms - ONE cory has signs of fin rot and has a cotton like patch on his head behind and above his eye.
b. Appearance of poop - normal
c. Appearance of gills - normal

14. a. Have you treated your fish ahead of diagnosis - no

Can you isolate the cory for treatment? You'll need a gram-negative medication like Marycin II - Furan2 or an antibiotic to treat it. Keep the lights off of the hospital tank while you're treating it. Rumor has it that it breaks down the medication.

I wouldn't treat the whole tank unless you see more of the other fish come down with the same symptoms.

I would check the kH and GH. In my sand only planted tanks they rose over time.

Other than that can't help much. Hopefully it is just the one cory.

What kind of cory is the sick one? Need to QT that one.
Also Corydoras are more sensitive to nitrates then other fish. Perhaps you should up the volume at water change time to 50%. On my cory tank I like to do 70-80% water changes. But I'm sure it's not necessary to do as much a change as I do but ..Hey It's on the breakfast counter by the sink so thats like 5 minutes from start to finish lol.

that sucks, just read a report that came out saying that my city and water supplier are in the top 10 WORST water qualities in the country. With Nitrates and Nitrates being at "illegal" levels. I can't get my Nitrates below 20ppm and now i may know why. i usually hit 40ppm right before a water change. I did just under 50% water change this time.

also i am starting to think i may be a little over stocked. maybe i should find a new home for the corys :/ would suck but they may be better off.

Yes.. It is a bit overstocked. But nobody will take your sick cory. And if the water where you live is like that then it's not going to any better for another local owner. You should test you tap and see how bad it is at your house. Also you could mix RO water 50/50 with your tap to help. And then start doing 50% water changes.. but try twice a week or more till you get the nitrates down. You don't need to go to the petstore for RO water you can get it at most grocery stores much cheaper. And actually yes with sand you vacuum but how you do it is run the syphon right over the top of the sand to get the waste ..much easier the gravel vacuum precess. As soon as you can go get prime water condtioner. It will detoxify the nitrate problem you are having for24-48 hours(and nitrates if they are in the tap). With it being so high from the tap and in the tank I would dose the whole volume of the tank as opposed to the vume you are changing out. And don't forget to QT you cory and treat #1 with clean water *VERY* clean water..So 50%RO 50% tap and as Romad said a gram negative medication.
