Unhealthy fish and/or water

I woke up this morning and noticed that one of my red eye tetras (the one i always called loner for obvious reasons) eyes are popping out of his head, It also kind of looks like his nose is getting smaller. There was also a feathery like substance on my driftwood. Any ideas or suggestion? HELP!
