Help needed to identify diseased Devario

I am facing disease problems with Giant Danio with red spots in the scales and caudal fin breaking as posted in photos as attachment. Plz help in identifying the disease and possible treatments.

I have treated them with antifungals and Kmno4 but it does not help and the fish eventually dies. These disease occured after keeping the fish more than 3 months in my aquarium.. I give regular water change (50%) once a week with water pH 6.3-6.8 and hardness below 95.
