I have seen similar worms in all my tanks at one time or another and never worried about them. Some are planaria, some are just the odd worm that gets in the tank from a fly, midge or other insect. I have never seen an ill effect one my fish nor snails from them.
If it's planaria this is caused by overfeeding. Cut down on feeding a bit, maybe change to every second day rather than every day if that's what you currently do for a few weeks, and do a thorough gravel vac which should get rid of most of them. Planaria are little white thread like worms, in case you were having trouble identifying them.
