Can anyone tell if this is a disease or a possible bite wound? The fish isn't looking good.

Hard to say, could be a wound almost healed, or beginning of possible bacterial infection.
I might review water parameter's,diet, and look for any aggression from tankmates as well as remove any sharp object's that fish could flash against to cause damage.
Some, might try twice weekly water changes for a few weeks while seeing that the fish received proper diet for African cichlids which should include substantial alage matter, or vegetable matter.
Food's such as spirulina pellet or flake, in addition to quality Cichlid pellet or flake such as Hikari Bio'gold,and or New Life Spectrum are favored by some.
Other's may choose to isolate the fish if condition worsened, and treat with medication such as Maracyn after removing any carbon from filter(s).
