VERY sick angel!
 

Each time I feed my angel fish, he starts jerking about and coughing up all his food. You can tell he's starving, because he comes up to the surface each time I approach the tank, which he always does when he's hungry.

Anyway, yesterday I noticed a small hole on one of its gill covers... HELP, I've looked all over the internet.. And I can't find any disease which matches this description.

Fish will never know when they are often full which is why it is the aquarist's responsibility to control their appetite.:)
It seems your fish may have bacterial gill rot. Any medications treating against bacterial infections will do the job. Where are you located? Countries have different med stocks available. Maracyn-2 is one of the meds that will treat bacterial infections.

What other fish are in the tank? Not all meds are safe for all fish. Also, what are your water params? It is important to ALWAYS check water params before using any medications. Any ammonia or nitrite or nitrate over 40 can cause the medications to have an adverse effect on the fish. Also, what is your temp? Some medications will require extra circulation/oxygenation in the tank due to lowered oxygen content during medicating.
