Can you identify this worm? (Pics and Video)

I noticed this guy hanging out in my gravel today. He is a flatworm, about 1.5 inches long when fully extended, and about a quarter inch wide. He inches along like an inchworm, attaching at the back end and moving the other end forward, then attaching the front end and pulling the back end up. I managed to move him onto the glass for photos and a video. Any help in identifying him would be appreciated. I've had many fish deaths recently and wonder if he has something to do with it.
