I had to fabricate a large shroud around my filter intake because the same thing happened to me. Though the fish did not immediately die, he did so a few days later.

I made a large plastic shroud from a big syringe and drilled holes in it. The further from the filter I got, the more holes I drilled to space out the suction evenly across the entire shroud.

My suggestion to you, if you're not good with your hands, is to take a small drill bit and drill holes the entire way down your intake tube. This will decrease the nozzle pressure across the entire tube. Drill more holes the lower you go. This seems to work pretty well for me.

A healthy fish should not get stuck in the filter intake. I have fish that like to hover around my 350GPH intake and grab passing food. You can get a sponge "prefilter" to go over your intake, but most likely the fish was ill, which was the problem, not the filter.
