Are you gonna go with stems, swords, or any crypts? Or just mosses, anubais, and java fern? Also do you want an inert substrat were it doesn't lower pH or do you?

If no to noninert go with Ecocomplete or Flourite. If you want your substrate to lower your pH, there is all those stratums like Fluvals (Fluvals Stratum has little nutrients, so use root tabs!) or you can use Azoo Plant Grower Bed (Should be $31 shipped) this does have nutrients.

I wouldn't, crushed coral is going to make your hardness go through the roof. The only time you should add crushed coral or shells is to increase or stablize kh or gh, only a small amount is necessary. The only fish I would use crushed coral for is cichlids. Though it depends, is there salt in the corals? Will they provide salt to the water? I would do some tests to see.

Best bet is to sell it on Craigslist and buy a bag of play sand at home depot or Lowe's for low $5 for a 50lb bag. It won't raise your th or kh unless by a very very small amount. And it looks good.

Oh yes, tannins would be beneficial to help ward off any further bacterial infection. Once you have the oak leaves dried, just place a leaf in or crumble the leaf (your choice) into the water. Crumble/place as many leaves as necessary to get the water to turn a weak tea color. Tannins can be used with epsom salt/Stress Coat no problem. That's a good idea to phase him down to 1 tsp at least and see if he can handle just that amount. You're doing a great job with him. :) Such a lucky fish he is!

Sounds like his skin has shrunk to fit his face. Since he was really huge for a really long time, there is a distinct possibility he'll always have some swimming problems like you suggested. His organs were being pushed inward by his bloat/tumor/whatever and that in turn pushed them into his swimbladder. He can be on epsom salts pretty much all his life if that is necessary but if you want, you can try to phase him off them and see if it makes much of a difference. If he doesn't have a bacterial infection or fluids in his system, the epsom salts may not help anymore. If he has trouble, you might need to adjust his tank so that he has lots of tall plants to rest on near the surface. This is what I've done with my paralyzed betta who can't move his back end. He has a shallow tank with lots of plants that he perches in like a bird.
