Yes, he did pass on a few days after being medicated. I just today received a new betta, who is a smaller tank for a least a few weeks until I am sure of his health.
This new guy is much more active, and I recieved him while he was still in the box from the petstores new shipment. This way I can take proper care of him instead of him sitting in the petstore being neglected.

Hey i was worry and waiting for update about your little guy, but i am assuming he didn't survive. I just want to say that you tried your best and at least his last few days he was loved and in the clean water which is more then store can give him. And please don't get discourage , it really happened a lot in the fish world . All of us even who has bettas for long time had the same sad experience. And we all learning..I just hope that you will decide to rescue another betta. If you will make sure you disinfect the tank because whatever he had is contagious for another fish. Please let me know if you need any disinfection instructions. I really think your little gut had something from beginning , so he came sick:(
