Your nitrates are WAAAAY high. What is 'safe nitrite'? If it isn't 0 it's not safe.
They should ALWAYS be -
Ammonia 0
Nitrite 0
Nitrate >20

You need to do a 50-75 WC.

The nitrites are at 0. Sorry, thought I wrote that next to "safe". Will do.
