I am going to ask alot of questions. Sorry ahead of time.

Do you use de-chlorinater? What kind of plants do you have? Is your filter working properly? Good aeration? Do you have a heater? Have there been any fluctuations in pH, tempreture, hardness, etc.? Are you useing and replacing your filter media (if any) properly? Are the fish showing any signs of fungus, white patches, spots, dots, rips on fins, patches on mouth, etc.? How much are you feeding the fish a day? What do you feed them?

The questions will help us, help you. Thanks!

first off, hello and welcome, secondly, sounds like cycle trouble to me.

when you set up a new tank, hide an extra
sponge or two behind some decor, that way you have
something seeded for you next filter.

Your problem is right here-
ammonia 0.25 p h 8.2 nitrite 0 nitrate 0

You need to do daily water changes until ammonia and nitrite are 0.

If this is impossible, then try adding Ammo-lock or Zeolite to the aquarium to neutralise the ammonia.
