You could perform surgery I guess...
Get some damp CLEAN cheesecloth, net her out, wrap her in it, and pull it slowly and gently out with tweezers. Don't worry about being gentle for the fry, it's probably dead anyway...
Actually, the mother is probably doomed ragardless if the fry stays like that much longer, and probably doomed from the stress of the complication plus stress of surgery.
*shrug*
The technique with the cheesecloth is something I picked up on a site for artificially inseminating guppies.. (odd, huh? basically rub the male, and squirt his stuff in the female. I wouldn't do it, but apparently one person in the world has done it successfully.)
