Water parameter's sound good.
I might look at the pleco's belly to see that it is full looking as opposed to sunken or concave, which if the latter,,could be indication of internal parasites.(treat accordingly)
Might also consider offering some shrimp pellet's ,carnivore pellet's,or a sinking type cichlid pellet food such as New life spectrum or Hikari cichlid bio -gold to add a little bit of roughage, and animal protein.
Would not offer too much, but maybe once or twice a week along with the other foods you mention, and I would offer the pellet food of an evening after the light's have been turned off so that other fishes don't eat it all up before pleco can feed.
Common pleco is omnivore and will eat a little of about everything,could be it's getting plenty of vegetable matter but maybe not enough other foods.
Would not be in a hurry to treat the fish with meds unless as mentioned, the belley of the fish appears to be sunken instead of full, and would treat the fish in seperate quarantine tank or plastic tub.
Might try offering foods mentioned, and upping the weekly water change to 50 percent and see if foods mentioned don't bring some color to the waste produced before considering medication's.
