What's wrong with my banana plant and corckscrew val?

So I bought some banana plants and corckscrew vals for my 5 gal tank that has an overhead light that's 15w 6500k bulb and gets fertilizer once a week. They looked like the were doing well but now are slowly dying off. I thought maybe it was the switch to my tank shocked them but they're not showing any new growth and are just losing color. Any ideas would be great.

If you can post some pictures of the plants, it will help in determining what might be going on.

Yes, and some data too. Water parameters, how long have you had the plants, how long has the tank been running.
