i am guessing that you don't enough sand to hold your hair grass. and we plant hair grass into bits. you have to divide them into smaller clumps then use tweezers to plant them on the substrate. then cut the trim them into half.

as for your slime problem, if you have a driftwood in the tank, the slime probably came from it. you probably forgot to boil it. if there are DW on the tank, then it could be some fungus as what geomancer said.

hairgrass, is bit tricky plant :D
