Correlation doesn't necessarily mean causation...

What is the tank size, how many fish, what kind of fish, what is you water change routine?

That's a pretty high bioload for a 120... but with the pangasius in there everything else is on borrowed time (possibly even the tank itself, but that's another story).

How often do you do your water changes for regular maintenance? How do you physically preform them and how much do you take out?
