I've heard gourami's are prone to(or at least sometimes get) hole in the head disease, and also wasting diseases.
It could be parasites and you may need a different med, or she could be stressed from other fish even if they're not acting aggressively towards him.
If you can, I'd move her to his own tank, feed her extra food, or just feed her small amounts a couple times per day. If she stays skinny and doesn't gain, and still eats, it's likely parasites. The way she looks does seem to be a nasty wasting disease, or parasites since you say she gets enough to eat but gains nothing. If she's had them that long and no others have been affected, it's less likely parasites, but still a possibility. Also possible something toxic hit her if she has a weaker system or was sick prior, so it's only affecting her.
TB and Hexamita are also a possibility. Less likely if there aren't any other issues aside from skinniness.
Can we get water parameters, tank size and tankmates, any medications used in the last two months or so as well? Any stringy or white poo?
I had a gourami that looked like this last year. Treated for parasites and bacterial infections...didn't do anything for him though. =( Just being skinny with nothing else is likely parasites, just need to find a med that works.
